It definitely varies. It's just like in undergrad some of my profs would walk around during tests and others would leave the room entirely.
I would recommend taking some PTs in a public place if only to get used to people being up and around you.

I would also vote to include it as well. There are no hard and fast rules about including volunteer work on your resume. Also, 3 months of volunteer work is a perfectly acceptable amount of time to feel comfortable including on your resume.
Nope, there's no difference between February tests. Often when people take a test and score highly they go into the next test with more confidence. I believe that in itself can lead to a score increase.
