I don't think you have a problem at all! Just tell your story. The admissions committees use résumés to get a big-picture overview of what you've been up to, and I don't think you have anything to be embarrassed about.

You'll definitely still be able to apply early enough to get scholarships! It wouldn't be a bad idea to begin working on your essays before the test, though—so long as you don't have to sacrifice study time.

It depends on the content of each essay, but I think it's always good if you can cut down on what you're asking the admissions office to read. If you cover your diversity factors in your PS, you don't need to write a separate DS.
