I just made another email account specifically for admissions/LSAC stuff. It took 2 minutes and now all my law school stuff will be contained within this one email account.

As a URM, specifically AA URM, you def have a shot at H 3.5+ and ~167+. I think you have a shot at Stanford with those numbers, too. Yale does what Yale wants because it's Yale and they do Yale things at Yale because it's Yale

Only the grades from undergrad are used to calculate your LSAC GPA. If you received those grades after you completed undergrad, then they will not be included in your LSAC GPA. You still have to send in the transcripts though.
