I think it helps to give them the LSAC form even if they're going to submit it electronically just because they tend to have large numbers of papers and electronic documents to deal with. A resume is great, and you may also want to give them a copy …

My undergrad has a website like that as well, but I get cold feet despite knowing I'm going to be glad I asked them. This is helping to build courage, so thanks for sharing your ideas.

Been there before--reading it a few times, making little changes, reading more, then finding mistakes in the sent version. So I get it. One trick that could help is sending it to yourself first.
